1. ERAU Email Address
   Use this line to enter your ERAU email address. If you are a new user of the lab, the system will ask you for your first and last name as well to complete your RPL account. Please note that all communication regarding your print request will be emailed to you at the provided ERAU address.
2. Requester Position
   Use this field to denote if you are a student or a faculty member. Please note that the “Other” option is never to be used unless you have a specific reason, if so please specify in the description area of the request.
3. Project name
   Use this field to give your project a unique name so that it can be easily identified. When you come to the lab, please know both your project name and ticket number.
4. Mobile Phone
   We will only use this number if we need to contact you on an urgent matter.
5. Department
   We need to know which cost center your parts should be billed to. Select "Other" and include your club name, group identifier, or research project name. Capstones: select COE or "Other" if outside the COE. You will receive a Quoting Document in response to your ticket. You can also note your billing code if you know it.
6. Class Level
   This field is used to auto-prioritize tickets based on class level. If your print request is not associated with a class, please select “Other” or "URI".
7. Class Identifier
   Most print requests are in association with a class. Please use this field to put down the class number.
8. Approving Faculty Member
   Use this field to denote what faculty member approved your print. If the print request is for a class please put down your professor.
9. Desired Completion Date
   If you need your parts by a specific date, please put that date down and we will do our best to meet your deadline. If you need the part as soon as possible, leave this section blank. This does not override our SLA Policy.
10. Desired Material 
    Use this field to request the specific material you would like to be used for your part(s). If you're not sure, select "Not sure, help me decide" and a technician will help you make the best choice based on your part design and use case. 
    1. Desired Machine
       Use this field to request a specific machine you would like to be used for your part(s). If your not sure, select "Not sure, help me decide" and a technician will help you make the best choice based on your part design and use case.
    2. Desired Color
       Use this field to request a specific color for your part(s). If you have no preference, select "No Preference" and we will use the color happens to be currently loaded in the machine.
11. Description 
    Fill out a detailed description of how your parts will be used, if you plan to use fastening hardware of other non-3D parts please describe the use case scenarios of each.
12. Attach a File 
    Use this area to attach your file(s), if you have too many files for our system please give us a link to a shared folder location in the description, or bring the files by the lab on a USB during office hours.
